How Insurance Billing
Actually Works

Mental health insurance billing has its own language and process. Here's a plain-language breakdown of what to expect from start to finish.

Verify Your Benefits

Before your first appointment, we'll run an insurance verification to confirm your coverage, copay or coinsurance amount, deductible status, and whether a referral is needed. We'll share what we find with you. You should know your benefits before you start therapy with any provider.

Your First Session

At intake, your therapist conducts an assessment and assigns a DSM-5 diagnosis code required for all insurance billing. This diagnosis becomes part of your insurance record — something to consider when choosing between insurance and private pay.

We Bill Your Insurance

After each session, our billing team submits a claim directly to your insurance carrier using your therapist's NPI number and the appropriate CPT procedure code. You don't have to do anything.

You Pay Your Portion

Once the insurance processes the claim, you'll owe your copay, coinsurance, or any remaining deductible balance. We collect your portion at the time of each session or invoice you after the claim is processed.

⚠ Important to Know

Insurance verification is an estimate, not a guarantee. Final coverage is determined when your insurer processes the actual claim. We encourage all clients to call the member services number on their card to confirm their mental health benefits before their first session. You will be responsible to pay any fees your insurance does not cover. It is your responsibility to know your benefits and inform the practice of any insurance coverage changes.

◈ KEY INSURANCE TERMS EXPLAINED 

Decoding your 
Insurance Benefits

Understanding these terms will help you know what you'll actually owe before you ever walk into a session.

Premium

The monthly amount you pay to keep your insurance active — separate from any therapy costs. You pay this regardless of whether you use your insurance.

Deductible

The amount you must pay out of pocket before your insurance starts sharing costs. For example, with a $1,500 deductible, you pay the first $1,500 of covered services yourself each year.

Copay

A flat fee you pay per visit — for example, $30 per therapy session — after your deductible is met. Copays are predictable and easy to plan for.

Coinsurance

Instead of a flat copay, some plans charge a percentage — e.g., you pay 20% of the session fee, insurance pays 80%. This kicks in after your deductible is met.

Out-of-Pocket Maximum

The most you'll pay in a given year. Once you hit this ceiling, insurance covers 100% of covered services for the rest of that plan year.

 

In-Network vs. Out-of-Network

In-network means your insurer has a contract with the provider, resulting in lower costs for you. Out-of-network providers may still be partially covered under your plan, but usually at a lower rate.

Prior Authorization

Some insurance plans require approval before they'll cover certain services. We'll let you know if this applies to your plan and help facilitate the process.

EOB (Explanation of Benefits)

A statement from your insurer (not a bill) that shows what was claimed, what they covered, and what you owe. Review these to catch any billing errors.
